New Patients
Click here to pre-enroll and save time when you need an appointment

We are accepting new patients. To provide patients with low wait times and convenient care, we encourage prospective patients to pre-enroll with EP Health & Wellness, PLLC. Pre-enrollment is a simple process where patients complete insurance and health history paperwork at their convenience right from the website prior to their appointments. This simple process helps reduce wait times when care is needed. To have a staff member reach out to complete the pre-enrollment process, please click the link above and complete the patient form.

All appointments are scheduled in advance with adequate time for your needs. We will make every attempt to honor your appointment, for we do understand the value of your time. Please understand that unpredictable delays may occur. If timing is critical, you may reschedule. If you have to cancel your appointment we ask you notify us as soon as possible.

Telephone Calls

Patients are encouraged to call or text anytime they need medical advice or have a simple question. In order to provide the best quality of health care we do ask patients to call our triage nurse during regular office hours when your chart is available for reference. If our nurse is assisting another patient, please leave a message including your name, phone number and best time of day to return your call.

If you do not have refills left on your medications, ensure you make an appointment at least 2 weeks prior to running out of your medication, to ensure you get an appointment that is convenient to you and to ensure you do not run out of your medication. If your refills have expired, you must come in for an appointment to receive additional refills.
